It was the mirror on a Honda minivan! I was expecting it to be one of those outrigger mirrors like on a truck. So the van was only about 6 inches away from the guy. A-hole.

Cyclist Struck Sunday near Carlisle
"Ford was the lead bicyclist in the group, police said. After McLane's mirror hit him, Ford lost control of his bicycle ad hit the passenger side of the van before crashing to the road. Ford, who was wearing a bicycle helmet, suffered what police said were moderate injuries.

I would call this a "template error". Reporters are so used to writing about car accidents that they cannot tell a person from a car if they have the same name. (That said, I don't have any serious complaints with the content of the article.)
